We are authorized to collect personally identifiable information (PII) from you by our licensing as a life and health agent of the State of Texas as well as Illinois and a Certified Marketplace Broker with Healthcare.gov. Any PII we collect is used to;
Determine if you are qualified to enroll in a QHP
Determine if you are eligible for any APTC to assist you in purchasing a plan
Assist you in fully completing an application for Health, Life, Dental or Supplemental coverage
If you choose to give us PII, we may share this information with only the relevant insurance company that you have authorized us to help apply for coverage with and in the cases of consumers being helped with a Healthcare.gov application your information will also be shared with CMS (a federal agency) and that record will be maintained in the federal System of Records. PII is used or disclosed only under the following circumstances: Your explicit intent to apply for an insurance option.
Providing us with PII is completely voluntary. If you chose not to provide us with the PII requested, or not to respond to certain questions, then you may experience difficulty in one of the following: completing your insurance application, having your application accepted by the carrier, having healthcare.gov not be able to determine your full qualifications when it comes to; enrollment, APTC (subsidies) and/or Cost Sharing qualifications.
